How it works
We know a care arrangement is often needed quickly. Our intake team is ready to respond, assess and match, so you can focus on the young person.

We review the care arrangement referral, assess suitability, and identify the care arrangement and care team best suited to the young person’s needs and location.
We confirm the care arrangement, agree the transition plan with you, and prepare the home and care team ahead of the young person’s arrival.
Once the care arrangement commences, the young person is supported by a consistent care team, with regular communication and reporting back to the Department and the child’s care team.
